The pooled results of two very large randomised controlled trials comparing aspirin with placebo concluded that medium dose aspirin (160-300 mg) started in the acute phase of an ischaemic stroke produces a small net benefit (13 fewer patients per 1000 dead or disabled).95 Whether this benefit arose from an effect on the stroke itself or simply through earlier initiation of secondary prevention of stroke and other thrombotic complications is uncertain. We therefore start all patients on aspirin 300 mg as soon as a CT has confirmed an ischaemic stroke unless there is a specific contraindication; we later discharge patients on a maintenance dose of 75-150 mg per day.
