Physical Examination

Patients who have a triceps tendon rupture initially experience sudden acute pain along the course of the rup-

The examiner squeezes the triceps muscle belly and observes the elbow for extension motion. If none is present, a complete tear probably has occurred.

tured tendon. Ecchymosis may be present. If an avulsion fracture is also present, the proximal olecranon may be tender. With a complete rupture, elbow extension is either weak or absent. A palpable defect may be felt in the tendon, but is not always present.40 With a partial rupture, elbow motion and extension against resistance may be maintained.38 Viegas writes that the "modified" Thompson test is helpful in making the diagnosis of a complete tear31 (Fig. 9.11).

In the case of a distal triceps tendon rupture, unlike that of a distal biceps tendon rupture, radiographs of the elbow should be obtained because they frequently demonstrate calcific or bony fragments.33,34,36-38 Radiographs also enable the examiner to determine the presence or absence of a fracture. A nondisplaced avulsion fracture may indicate a partial tear.38
