Agonist High, constant Initial pituitary and gonadal stimulation followed by suppression within 2 weeks Antagonist Constant Pituitary and gonadal sup pression and include hot flashes (sudden intense surface temperature elevation and sweating), dry skin and vagina; long-term use may decrease bone density. The addition of estrogen and progesterone can reduce the adverse effects while maintaining gonadotropin suppression. However, there is a continuing need to address the recent cancer risk cautions issued for short-term versus long-term use of estrogen-progesterone combinations as hormonal replacement therapy.
