In this chapter, we provided an overarching process for the clinical assessment of personality disorder pathology among older adults. We described several strategies and examined potential complications with older patients. We offered these guideposts with an important theme in mind: Accurate patient assessment is crucial for case formulation and subsequent treatment planning. We concur with the astute observation reported by Mroczek et al. (1999) that "there is a uniqueness about personality disorders in late life that warrants special measurement" (p. 136). Diverse combinations of the approaches and instruments we suggested, in conjunction with a thorough assessment of episodic mental disorders and attention to the unique context of aging and coexisting medical conditions, will likely provide the rich clinical data that can propel the treatment process with older patients.
