Adjunctive therapies that may be helpful include: the aggressive treatment of obesity, the maintenance of nasal patency, the avoidance of androgens, the avoidance of sedatives (including alcohol), and the treatment of underlying disorders such as hypothyroidism and acromegaly. Modafinil may play a limited role in patients persistently sleepy despite adherence with CPAP therapy. Further work to better define the basic neurophysiology of sleep apnea may lead to novel pharmacologic therapies that may effectively treat our patients.
