The Classification and Differential Diagnosis of Erythrocytoses

The term erythrocytosis has descriptive merit for the nonspecific observation of an increased proportion of red cells in the peripheral blood, and will be used in this differential diagnostic section. Using this term rather than polycythemia avoids the incorrect assumption that "polycythemia" equates with PV. Ery-throcytosis defines the pathological change precisely and falls into line with other descriptive terms such as leukocytosis and thrombocytosis. It is more appropriate to define an erythrocy-tosis in terms of the hematocrit (Hct) rather than the hemoglobin (Hb) value since in the erythrocytoses iron deficiency may occur and then the Hct may be disproportionately higher than the Hb value.88 The Hct is therefore the best indicator of ery-throcytosis available from automated cell counts and, although

the upper limit of normal will vary between laboratories it will be around 0.51 for males and 0.48 for females.89 However, patients should not be further investigated until a minimum of two separate blood counts have been performed and both have shown a raised Hct. Patients with a raised Hct should be further investigated for an absolute erythrocytosis by measurement of their red cell mass (RCM). However, males and females with Hct values above 0.6 and 0.56 respectively, can be assumed to have an absolute erythrocytosis.90
