An increased risk of secondary AML has been associated with the use of alkylating agents as well as epipodophyllotoxins in children treated for a primary malignancy. Estimates suggest about a 5 percent increased risk of developing AML within a 5-year period.43 Secondary AML associated with epipodophyllo-toxin therapies occurs earlier (usually within the first 2 years following therapy) and is often associated with cytogenetic abnormalities involving the MLL gene at 11q23. In contrast, leukemias following alkylating therapies typically occur several years later, and are often associated with losses of chromosomes 5 and/or 7.43 These observations can help in developing hypotheses for future studies that investigate de novo leukemia (see Chromosomal Abnormalities below).
