Controlled studies have shown that outpatient withdrawal is safe and effective for mild and moderately dependent alcoholics. (45> Advice for patients withdrawing at home is given in Box 1.. Hayashida et al.'(4) randomly allocated 164 mild to moderately affected patients to either inpatient or outpatient detoxification. Completion was successful in 95 per cent of the former and 72 per cent of the latter; inpatient care cost eight times more than outpatient care.

Admission to hospital is indicated when the home social milieu is inimical to abstinence, or when there is a history of withdrawal convulsions or delirium; it is urgent when there are any signs of Wernicke's encephalopathy.
