Teams

The team concept is accepted as the key to effective psychogeriatric care. ^l6 Teams consist of groups of individuals—professional as well as non-professional—working together to support the elderly individual. These include the primary care team, community-based health-care teams including mental health workers, and hospital-based teams including members of the specialist psychogeriatric team. These teams should interact with each other smoothly, and participants may function as members of more than one team at once. Case management, with one worker responsible for co-ordination of care for the patient, ensures continuity of care and follow-through.
