Community mental health teams are increasingly aligning themselves geographically with primary health care teams and may in the future align themselves with primary care groups. This improves communication between the two teams, particularly if each has a lead contact. Both teams need to know how best to communicate key information. This may seem fundamental, but most inquiries after tragedies conclude that poor inter-agency communication had been causative. The teams need to understand each other's roles and responsibilities (aided by multiprofessional learning) and develop joint protocols for care.
