The target serum phenytoin concentration range is between 40 and 80 pmol/1 (but see Fig 2). Higher concentrations are associated with an increased risk of toxicity.

The blood sample can be taken at any time after the last dose. Because phenytoin kinetics are non-linear (i.e. the amount in the body increases disproportionately with increases in dosage) dosages should not be changed more than every few weeks and should be limited to changes of no more than 25 or 50 mg in the total daily dose.
