Patients who represent a continuing and immediate serious risk to the public may be detained either under a civil act or as a result of a hospital order from a criminal court with the purpose of treatment or the prevention of deterioration of their condition. In the United Kingdom, such people are usually contained within the Special Hospitals (maximum security hospitals)—Broadmoor, Ashworth, Rampton, and Carstairs. Medium secure units are usually reluctant to accept such patients directly, either because of the chronic and enduring nature of their condition and the lack of specialist facilities to treat these disorders, or because of the high level of dangerousness.

In various countries there are inpatient units for the treatment of sex offenders. This is usually based on community and cognitive- behavioural principles. In addition, patients are often admitted to general psychiatric units.

In acute psychiatric provision

There are rare occasions when a concomitant psychiatric condition such as severe affective disorder or psychotic illness is treated in the usual way as for any other patient suffering from such an illness.
