First, the presence of a significant physical disorder must be excluded. The illness must be distinguished from the following:

• paranoid schizophrenia

• substance-related disorders (e.g. itching due to alcohol-related liver failure, cocaine abuse, etc.)

• organic brain disorders

• severe depressive disorder with hypochondriacal delusions

• somatoform disorders, especially body dysmorphic disorder

• obsessive-compulsive disorder

• factitious disorder.
