Trichotillomania is currently classified as an impulse-control disorder in both DSM-IV (1) and ICD-10.(2) Such disorders are characterized by the presence of mounting tension before the behaviour and pleasure or relief during or after the behaviour. They are egosyntonic (that is to say they are consistent with the will of the patient at the time). Although most individuals with trichotillomania describe such traits, the breadth of phenomenological experience with trichotillomania has led to proposals that trichotillomania is better described as a tic or a habit,(6) an obsessive-compulsive spectrum disorder,(8) or an affective spectrum disorder.(9) Additionally, it has even been proposed that trichotillomania is not a disorder perse, but one of many behavioural symptoms of anxiety, depression, or a more general state of tension.(4,6)
