Vitamin K administration to patients on warfarin therapy

Effect of vitamin K is delayed several hours even with IV administration. Doses >2 mg cause unpredictable and prolonged resistance to oral anticoagulants and should be avoided in most circumstances where prolonged 522 warfarin therapy is necessary. Particular care must be taken in patients with prosthetic cardiac valves who may require heparin therapy for several weeks to achieve adequate anticoagulation if a large dose of vitamin K has been administered.
