Immediatetype hypersensitivity reactions

May occur soon (30-90min) after transfusion of blood/component. Antibody often unknown but in some cases is due to antibody directed against IgA (in recipients who have become sensitised).

• Mild reaction: urticaria, erythema, maculopapular rash, periorbital oedema.

• Severe reaction: bronchospasm.

Management

• Stop transfusion immediately.

• IV colloids to maintain BP/circulatory volume.

- epinephrine (adrenaline) 1:1000 1mL IM stat

- hydrocortisone 100mg IV stat

- chlorphenamine (chlorpheniramine) 10mg IV stat
