Weakness and cardiorespiratory fitness

Many people with MS will experience 'weakness'. In a survey of 656 people with MS in the USA, 63 had 'weakness or paralysis' which affected 45 of those with limitations on personal ADL.278 In the same survey, 77 had fatigue and 74 had 'balance problems'. In South Wales weakness was the commonest single symptom experienced (89 at any time, 80 at any one time).271 However, the term weakness may include many different phenomena and have many different contributing factors. The following two routines are basic workouts to build muscle strength. Choose the routine that is best for you based on the time available, your goals, your training experience, and your fitness level. Practitioners from a variety of disciplines, including dieticians, behavioral psychologists, and personal fitness trainers, are important components of a comprehensive weight management program. However, most of my obese patients either cannot or will not see these specialists. Typically, rural patients have only their local family doctor to help them manage their obesity. For the most part I serve as my patients' dietician, behavioral psychologist, and personal fitness trainer. Prevention Of Type 2 Diabetes

There is now unequivocal evidence that physically fit people are less likely to develop Type 2 diabetes and some intervention trials have shown that encouraging people with impaired glucose tolerance (IGT) to increase their physical activity significantly reduces their risk of developing diabetes (14-16). This benefit is independent of body mass index (BMI) and there is some evidence that physical activity has a greater protective effect as BMI increases (17). It may be of more importance for people at risk of Type 2 diabetes to increase their physical fitness rather than concentrate on weight reduction.

Cardiovascular Risk Factors

The role of exercise and the prevention of coronary heart disease in the general population have been well documented, but there is less evidence of a similar effect in people with diabetes. Modification of risk factors for CHD, including decreased total and LDL (low-density lipoprotein) cholesterol and triglyceride concentrations, have been demonstrated in Type 1 diabetes (20). People with Type 2 diabetes have two to four times the cardiovascular risk of those without diabetes and low cardiorespiratory fitness has been shown to be a predictor of mortality in men with diabetes (21).

The national recommendations for physicians to advise patients to be physically active are confusing. On the one hand, the Healthy People 2010 program recommends that by 2010 at least 85 of primary care physicians counsel patients about physical activity 4 . Also, the NHLBI Obesity Guidelines state, physical activity (PA) contributes to weight loss, both alone and when it is combined with dietary therapy, and that PA in overweight and obese adults increases cardiorespiratory fitness independent of weight loss 5 . The basis of cardiorespiratory training is to place greater demands on the heart (e.g., make the heart beat more times per minute) than what is required during rest. This results in a stronger heart that can pump more blood and deliver more oxygen to the body per heart beat, and a lower resting heart rate. Since most daily activities are aerobic in nature, improving the delivery of oxygen to the muscles will improve your work performance. (See Fuel Used During Exercise on page 26.) So, view your heart as an aerobic muscle that must be conditioned for optimum functional health and fitness throughout your life.

In asexual populations, the genetic drift is increased and results in an accumulation of mutations, designated Muller's ratchet. Aerobiccardiorespiratory Exercise

A cardiorespiratory exercise prescription generally consists of four areas frequency, Intensity, Time or Duration, and Mode or Type of Activity, often referred to as the FITT principle. Recommendations in these different areas are dependent upon the goals of your exercise program. The following recommendations, suggested by the American College of Sports Medicine, are geared toward 1) enhancing physical fitness, 2) reducing risk for future development or recurrence of cardiovascular disease, and 3) ensuring safety during participation in exercise.

Intensity of Exercise

Measuring increases in heart rate during a workout is a quick and easy method to gauge the intensity of your workout. To measure your heart rate follow the instructions in Figure 5-1. Once you measure your exercising heart rate how do you know whether you are exercising at the appropriate intensity Depending on your age and fitness level there is a target heart rate zone that is appropriate for your exercise intensity. Use Figure 5-2 or Worksheet 5-1 to find your target heart rate zone. See Training Design and Progression on page 33 to determine what heart rates, within this range, you should aim for during exercise based on your level of physical fitness and your fitness goals.

Beginning a Running Program

When starting a running program, combine walking and jogging. Gradually increase the time spent jogging and decrease the time spent walking. Remember that your exercise intensity should be between 60 -75 of your max HR, so adjust your pace accordingly. Table 6-2 outlines a beginning jogging program to help make your transition easier. Advance to the next phase once you can consistently perform the walk-jog cycles outlined within your target heart rate zone. If you are interested in running for fitness, a good goal is 6 to 8 miles per week, spread over 3 running days of 2 to 3 miles each. Start a running log to track your workouts (Worksheet B-1), noting mileage, time, heart rate, and perceived exertion (see Chapter 5).

Increasing Your Running Workout

Twenty to 30 miles per week is a good training distance for an intermediate runner (Table 6-3). As a rule, your risk of injury sharply increases as your running mileage increases. So, if running for fitness rather than competition, keep your weekly mileage below 30 miles. Beyond this, your injury risks far outweigh any additional fitness benefits. Cross-train to work on aerobic fitness without running more than 30 miles.

The two other species that O. moubata was reported to have transmitted were B. hermsii and B. parkeri, which are genetically distant from each other and from B. duttonii (Marti Ras et al., 1996) (unpublished studies). Interest in discerning protein structures through prediction (PSP) is widespread and has been previously addressed using a variety of techniques including X-ray crystallography, molecular dynamics, nuclear magnetic resonance spectroscopy, Monte Carlo analysis, lattice simulation, and evolutionary algorithms (EAs) in energy minimization. We have employed EAs such as the simple genetic algorithm (GA), messy GA (mGA), and the linkage learning GA (LLGA), with associated public domain software. Parameter Estimation Using Swarm Intelligence

The algorithm simulates individuals flying through the search space. On each iteration, the individuals are separated into a set of leaders and a set of followers, based on their fitness. The followers use the locations of the leaders to change their flying direction, that is, search velocity. The evaluation of the biological activity of pharmacological relevant substances is based on the application of cultured cells as sensor elements. Among suitable cell culture models competitive analytical methods are needed to detect cellular reactions as a function of pharmacological substances. The cell shape is a well-known criterion to analyze the cell fitness due to the fact that cells react very sensitive on changes of their environment. The method of electrical cell-substrate impedance sensing (ECIS) is a new electrochemical application to analyze changes in the cell shape as a function of biological, chemical, or physical stimuli without invasion of the cells.11'12 Cell-cell and cell-substrate interactions can be characterized independently. Thus, the change of the cell shape is interpreted by electrical parameters.

Calisthenic Guidelines

When performing calisthenics to develop muscle strength or endurance, you should follow the same recommendations outlined in Chapter 7. Intensity is largely based on the number of sets and reps, and the length of rest periods. Resistance is provided by body weight rather than an external resistance. Proper form for calisthenic exercises follows many of the general exercise guidelines outlined in Chapter 7, including muscle balance and exercise order. Any of these exercises can be added to your gym-based routines to create more variety. The first that examined the effectiveness of a water exercise training program found positive effects upon pain, energy, social and sexual function as assessed by the MSQOL-54, and tension, fatigue and vigour, assessed by the POMS-SF.452 However, the second RCT that examined an exercise programme using a leg cycle ergometer found no improvement in either the physical fitness indices assessed or grades of fatigue, as assessed by the Fatigue Severity Scale.453 The last RCT examined the utility of lectures on exercise philosophy combined with nutrition, stress management and an individualised exercise programmes.451 The results showed no effect on either graded exercise time or on EDSS scores.

The Impact of Physical Activity on Health

Risk (RR) of all-cause mortality due to low fitness was 1.52, while smoking was 1.65, and for women it was approximately 2.0 for both low fitness and smoking 7 .
