A 20-year-old male basketball player presented with an 8-month history of mechanical pain and popping in the left hip following a fall in which he landed directly on the lateral aspect of the hip. Radiographs revealed no acute bony changes (Figure 2.9A). MRI on three previous occasions each showed signal changes in the medial aspect of the femoral head (Figure 2.9B).
Arthroscopy revealed a large full-thickness unstable flap of articular cartilage, displacing from the medial side of the joint, corresponding to the area of signal changes on the MRI (Figure 2.9C). Excision resulted in alleviation of his symptoms (Figure 2.9D).
