Details of each patient recorded include first and last names, address, date of birth and the prescribing doctor's name with specific worksite address. The type of doctor is also recorded— general practitioner or specialist type. National health identification (NHI) numbers have been introduced but are not yet complete and are insufficiently reliable for accurate statistics. In addition, many practices cannot identify patients by the NHI number and so it is essential when sending questionnaires to continue to use personal information to identify patients.
