Once meningitis is suspected, treatment must commence immediately, often before identification of the causative organism. Antibiotics must penetrate CSF, be in appropriate bacteriocidal dosage and be sensitive to causal organism once identified.

Initial therapy (before organism identification)

Neonates (above 1 month) - ampicillin, + aminoglycoside and cephalosporin Children (under 5 years) - ampicillin, + cephalosporin

Adults - penicillin G, or cephalosporin

472 Immunocompromised patient - ampicillin + cephalosporin
