Valve and aortic surgery

Valve surgery may be technically difficult and involve several valves or additional coronary grafts. Aortic valve dysfunction may be associated with aortic root abnormalities or aortic dissection. Many patients are now presenting for repeat surgery. All but the most straightforward valve and aortic surgical patients are likely to require significant postoperative ventilatory support.

These patients frequently undergo very long bypass procedures and encounter severe hemostatic derangements and respiratory complications. Phrenic nerve damage may occur during surgery but may only be identified when weaning is attempted. Non-invasive ventilatory support, such as nocturnal nasal positive-pressure ventilation, should be considered.
