Sadly, it is exceptional to find recoverable bowel; the most common situation that the emergency surgeon encounters is a major infarction of the midgut loop. If the patient is otherwise recoverable, it is worthwhile resecting large lengths of ischemic bowel and, following correction of metabolic deficits, restoring intestinal continuity. The capacity of the mucosa to regenerate was quite unappreciated before the advent of total parenteral nutrition because, at that time, all the patients died. Provided that nutrition can be maintained for a few weeks, many people with only a few centimeters of bowel remaining will eventually become able to feed themselves, although some will require lifelong parenteral support. Clearly, the outlook for patients who have lost their bowel because of trauma or strangulation will be much better than for those with underlying vascular disease.
