A subxiphoid pericardial window may be performed through a small incision at the left lateral side of the xiphoid cartilage. The diaphragm is reflected inferiorly, the pericardium is exposed, and, if blood or fluid is present, a 4 cm * 4 cm window of pericardium is excised and the perforation of the cardiac wall repaired. The pericardial sac may be approached through the standard left anterolateral thorocotomy at the fourth or fifth left interspace. Inspection, palpation, and ballotment of the pericardium may reveal significant amounts of pericardial fluid under pressure, which may be drained by a pericardial window.
