It is generally accepted that the initially sterile pancreatic necrosis secondarily infects with micro-organisms translocating from the gut. In a recent clinical study
(Luiten...etai 1995) it has been shown that SDD reduced the infection-related morbidity (35 per cent) and mortality (22 per cent) in severe acute pancreatitis. There was no difference in the early mortality from multiple organ failure associated with the acute pancreatitis, but late infection-related multiple organ failure and death was reduced by SDD. The number of patients with infected necrosis and the number of laparotomies per patient were significantly lower in the group treated with SDD group than in the control group.
