In this case the concern is the source of the air, which should not be from the ICU environment. The room should be pressurized with respect to all other areas. The air-flow and unidirectional requirements are the same as for source isolation. Unless the ICU is dedicated to treating severely immunocompromised patients, it is not necessary to filter incoming air to operating room standards or higher, provided that it has been drawn from the top of the building with the design considerations discussed above.
