Metronidazole is active against anaerobes, including Bacteroides species and Clostridia, and against parasites. It is as well absorbed by the oral or rectal route as intravenously and penetrates all tissues. Metronidazole should not be used in pregnancy. It is used to treat anaerobic bacteremia, intra-abdominal sepsis, and brain abscess. Aspiration pneumonia can be treated by metronidazole with a cephalosporin, but clindamycin might be more effective. Oral metronidazole is used to treat mild to moderate antibiotic-associated colitis. Giardiasis and amebiasis are other indications. The dose should be reduced in severe renal or hepatic failure. Peripheral neuropathy may develop during prolonged courses.
