Circulatory overload occurs particularly in patients with underlying cardiovascular disease or conditions predisposing to hyperviscosity (e.g. acute leukemia, multiple myeloma). Transfusion may precipitate complications of hyperviscosity, such as endorgan hemorrhage, including retinal and cerebrovascular bleeds. Transfusion should be undertaken with caution; leukopheresis may be indicated in acute leukemia before red cells are given.
