Phosphate replacement strategies must take into account the etiologies of the deficiency, the level of deficiency, and the side-effects of phosphate replacement. A strategy is outlined in Table.5. Complications of replacement therapy include hypocalcemia, hyperphosphatemia, hypernatremia, dehydration, renal failure, hypotension, phlebitis, and metastatic calcification. Therefore care must be taken when using replacement therapy in patients prone to these side-effects. Other electrolytes should be monitored closely during replacement therapy.
