Lambert Eaton myasthenic syndrome

This is a disorder of neuromuscular transmission where the primary defect is an autoimmune interference of presynaptic voltage-gated calcium channels which control acetylcholine release from the nerve terminal (L.eD..n..0.n eL&L 19.9.5.)- There is a deficiency of acetylcholine release giving rise to muscular weakness but, rather than fatiguing, the muscle power improves with activity. An antibody has not been identified as yet, but the condition is improved with plasma exchange and immunosuppression. About 50 per cent of patients with this condition have an associated neoplasm, usually an oat cell carcinoma of the lung.
