Key messages

• In adults, NO is indicated for patients with acute respiratory distress syndrome with severe hypoxia, patients with severe pulmonary hypertension, or patients undergoing heart transplantation as a test of the reactivity of the pulmonary vascular bed.

• NO is generally administered to mechanically ventilated patients on the inspiratory limb of the respirator either continuously or during the inspiratory phase.

• Inhaled NO is used at 5 to 15 ppm which can be accurately monitored by bedside chemiluminescence apparatus. At these concentrations of NO, nitrogen dioxide (NO2) and methemoglobin are usually well below toxic levels.
