Intraatrial reentry tachycardia

This tachycardia is considered separately because the re-entry circuit is confined to the atrium and does not involve either sinoatrial or atrioventricular nodes. As a result, carotid sinus massage may cause an increase in atrioventricular block but will not terminate the tachycardia. However, termination can occur with atrial or esophageal pacing since it is a re-entry circuit. Acutely, an increase in atrioventricular block can also be obtained with calcium antagonists, digoxin, or b-blockers. Class iA or IC antiarrhythmics may be helpful on a more chronic basis.
