Incidents and accidents with heated humidifiers

The absence of inspiratory temperature monitoring exposes the ventilated patient to a risk of tracheal burning or hyperhydratation. Moreover, bacterial contamination of the ventilator circuits occurs rapidly. The part of the circuit between the patient and the Y-piece is the most likely to become contaminated, after which contamination of the inspiratory and expiratory circuits occurs rapidly (33 per cent after 2 h, 64 per cent after 12 h, and 80 per cent after 24 h). Some teams place antibacterial filters between the ventilator and the inspiratory circuit and between the expiratory circuit and the ventilator. The tubes must be changed regularly (every 2-5 days), and ventilators must be sterilized between each patient.
