Multiple organ failure presents the paradox of widespread activation of the immune system combined with an increased susceptibility to infection which may itself contribute to continuing organ failures. The challenge is to develop therapies which enhance normal host defenses while limiting the self-destructive component. Glutamine is a semi-essential amino acid which is required for lymphocyte proliferation and cytokine expression, macrophage activity, gut mucosal function, muscle homeostasis, and renal function. Deficiency in body stores occurs rapidly in critical illness, and may be associated with increased infection rates; the same phenomenon has been reported following marathon running. Supplementation with intravenous glutamine may reduce mortality from critical illness. Arginine also has an important role in host defenses, and has been shown to reduce infection-related mortality in animal models. Impaired production of anti-inflammatory cytokines such as IL-10 may be as important as overproduction of proinflammatory cytokines such as IL-6.
