Cardiac function can be compromised in several ways, and the use of inotropic and vasoactive drugs is commonplace. Inotropic support of the circulation may be required because of other maneuvers, such as fluid restriction or the institution of positive-pressure ventilation. The effects of the condition itself, namely elevated pulmonary vascular resistance with consequent right ventricular strain and dysfunction, and the resulting leftward shift of the interventricular septum also impair left ventricular performance.
