Donors should be brainstem dead, preferably under 50 years old, adequately perfused and hydrated, and artificially ventilated. They should be free from hepatitis antigen and HIV, malignant disease (except primary brain tumor), systemic infection, diabetes mellitus, ischemic heart disease, and cardiac murmurs. There should be no family history of heart disease and no history of heavy smoking.

A chest radiograph and 12-lead ECG should be obtained. Myocardial dysfunction associated with brain death may be reversible and is not necessarily a contraindication to donation. Rarely, coronary angiograms may be requested by the transplant team in male donors over 35 years old and female donors over 40 years old.
