Drug techniques

Significant changes in the pharmacological management of pain are occurring due to a combination of improved understanding of the neurobiological basis of pain, advances in pharmacokinetics and pharmacodynamics, the clinical application of new drugs, and the use of new analgesic technologies. The current analgesic drug options consist of the following:

1. opioids;

2. non-steroidal anti-inflammatory agents;

3. local anesthetics;

4. adjuvants.

The choice of analgesia should be based on the location and type of pain (nociceptive or neuropathic), with allowance made for individual patient factors. Intravenous analgesia is used in unconscious patients, those with pain at multiple sites, and in the presence of contraindications to regional techniques. Intramuscular and subcutaneous routes are avoided. Regional techniques confer significant advantages in selected patients.
