High-dose aprotinin to prevent postoperative bleeding in cardiopulmonary bypass or to arrest bleeding associated with prolonged extracorporeal circulation requires a loading dose of 2 * 106 kIU (over 20 min) followed by 500 000 kIU/h intravenously. As a polypeptide, aprotinin is not active if given orally.

Low-dose aprotinin to reverse the action of thrombolytics requires a loading dose of 500 000 kIU (over 10 min) followed by 200 000 kIU/h intravenously until bleeding stops.
