HFV is a realistic ventilatory option in adult intensive care. Delivered at low pressure, it is an established management for airway trauma and persistent bronchopulmonary leaks. Considerable advances in technology now render it an effective option in ARDS, particularly in refractory patients, and in other conditions characterized by poor compliance. Enthusiasm remains sporadic but, with greater availability of the new technology, better understanding of underlying principles, and the further technical improvements envisaged, its application in adult practice is likely to increase considerably in the next few years.
