Pre-eclampsia is a major cause of maternal mortality and morbidity. Defective trophoblast development leads to failure of the physiological dilatation of the spiral arteries and relative placental ischemia. Release of an as yet unidentified factor from the ischemic placenta causes endothelial dysfunction in a variety of vascular beds and organs. The vicious cycle of neutrophil and platelet activation, endothelial dysfunction, and vasoconstriction leads to the progressive organ damage seen in pre-eclampsia.
