Critical illness can depress normal immune function in many ways, and this is influenced by the premorbid state of the patient, his or her genetic make-up, and drugs used in therapy. Although many endogenous inflammatory mediators are implicated, these agents are important in the normal functioning of the inflammatory cascade and it is only when the balance of pro- and anti-inflammatory responses is tipped too far in one direction that they become detrimental. As our understanding of inflammation advances, useful manipulation of soluble mediators can be attempted. Until then therapy must concentrate on adequate and early resuscitation and appropriate treatment of comorbidities, including renal failure and the nutritional state.
