Patients who repeatedly fail to wean pose considerable clinical and economic problems. In general, difficulties with weaning from mechanical ventilation arise when there is an imbalance between respiratory demand and respiratory muscle capacity. The major causes of failure to wean are listed in Table 1. Other causes are dyssynchrony between the patient and the ventilator due to inappropriate sedation or ventilator settings.
