Several tetanus severity assessment scores which are useful in management decision-making and prognosis have been developed. Neonates and patients with generalized spasms, cephalic tetanus, an incubation period of 7 days or less, an onset time to generalized spasm of less than 48 h, and more than 12 reflex spasms per 24 h generally have severe tetanus and consequently a poorer prognosis (Bleck and Brauner 1996). Intubation and prolonged ventilatory support are essential components of the management of severe tetanus.
