Traditionally, heparin is used with hollow-fiber hemodialysis and citrate with the centrifugation technique. With heparin, individual dosing is based on monitoring the activated clotting time every 30 min. However, an initial dose of 50 IU/kg and infusions of 25 IU/kg as necessary to maintain the activated clotting time at 1.5 times to twice normal is a conservative approach. In some patients, substantial amounts of heparin can be extracted by the procedure, obviating frequent reinfusion of heparin.
