Here the intensity of the radiotherapy beam is varied across the treatment field to ensure that a uniform dose can be achieved in a regular or irregular shape target while avoiding delivery of a high dose to the surrounding structures. Essentially, this may add to conformal therapy by further reducing the dose to sensitive structures.

Currently, 14 institutions worldwide specialize in this technology, prioritizing treatment of intracranial tumours and cancer of the head and neck and prostate. The beam can be produced by a modulatory accelerator or with the addition of a special collimator, but the technique is computationally complex.
